“Consistent with recent observations of increased e-cigarette use in the general population, our findings illustrate that e-cigarette use among tobacco-dependent cancer patients has increased within the past two years,” said co-researcher Jamie Ostroff from the Memorial Sloan Kettering Cancer Center in the US.

To examine available clinical data about e-cigarette use and cessation among cancer patients, the researchers studied 1,074 cancer patients who smoked and were enrolled between 2012 and 2013 in a tobacco treatment programme within a comprehensive cancer centre in the US. The researchers observed a three-fold increase in e-cigarette use from 2012 to 2013.

At enrolment stage, e-cigarette users were more nicotine dependent than non-users, had more prior quit attempts, and were more likely to be diagnosed with lung or head and neck cancers. At follow-up stage, e-cigarette users were just as likely as non-users to be smoking. Seven day abstinence rates were 44.4 percent versus 43.1 percent for e-cigarette users and non-users, respectively.

One person dies every six hours due to oral cancer in the country, signalling an alarming rise in the incidence of the disease, according to a top orthodontist.

The situation could be still graver as many cases of the disease went unreported, Secretary-General of the Indian Dental Association Dr Ashok Dhoble told PTI from Mumbai.

“Cases of the disease and deaths resulting from it in rural areas and among the poorer sections of society are hardly registered,” he noted.

He pointed out that with the high prevalence of smokers and widespread use of other chewable tobacco products, India has seen a steep growth in the number of oral cancer patients in the past decade.

In fact, Dr Dhoble said, oral cancer approximated to 40 per cent of all cancer-related disease in the country with the Northeastern states mostly affected.

“All Northeastern states are badly affected by the disease. The states like West Bengal, Andhra Pradesh, Gujarat and Tamil Nadu are also affected,” he said, adding every third person in these states used tobacco-related products.

Talking about measures to curb the prevalence of the deadly disease, he said that nothing short of a total ban on the use of tobacco was the only way.

“There is no other way to curb oral cancer… You have to ban tobacco in its every form,” Dr Dhoble said, pointing out that it was the nicotine present in the tobacco which made it addictive and difficult to kick the habit.

“Our government also understands the problem… But the huge number of people employed with the tobacco industry is the problem… The government has to provide them with an alternative livelihood and then ban tobacco totally,” he said.

He said a dentist was the first person to diagnose it since oral cancer was not just limited to teeth, but mouth, tongue and integral part of pharynx and gums as well.

The orthodontist explained that oral cancer was 100 per cent curable only if it was reported in the first stage. But once it slips into the second stage, the patient is left with a life span of just five years.

“I will advise people to visit a dentist and not to ignore even if there is a slight problem in the mouth… But above all they have to give up using tobacco in every form. That’s the best medicine,” Dr Dhoble said Stressing the need to educate people on the disease, he said that the Indian Dental Association, which will hold the FDI Annual World Dental Congress in Delhi in September, had been conducting awareness camps and counselling people for the last five years.

E-Cigarettes Won’t Help You Quit, Study Finds

Contrary to some advertising claims, electronic cigarettes don’t help people quit or cut down on smoking, a new study says.

Users of e-cigarettes inhale vaporized nicotine but not tobacco smoke. The unregulated devices have been marketed as smoking-cessation tools, but studies to date have been inconclusive on that score, the study noted.

“When used by a broad sample of smokers under ‘real world’ conditions, e-cigarette use did not significantly increase the chances of successfully quitting cigarette smoking,” said lead researcher Dr. Pamela Ling, an associate professor at the Center for Tobacco Control Research and Education at University of California, San Francisco.

These findings — based on nearly 1,000 smokers — are consistent with other studies and contradict the claims frequently found in e-cigarette advertising, she said.

“Advertising suggesting that e-cigarettes are effective for smoking cessation should be prohibited until such claims are supported by scientific evidence,” Ling said.

For the study, Ling’s team analyzed data reported by 949 smokers, 88 of whom used e-cigarettes at the start of the study. One year later, 14 percent of the smokers had quit overall, with similar rates in both groups.

“We found that there was no difference in the rate of quitting between smokers who used an e-cigarette and those who did not,” Ling said. There was no relationship between e-cigarette use and quitting, even after taking into account the number of cigarettes smoked per day, how early in the day a smoker had a first cigarette and intention to quit smoking, Ling added.

However, the researchers noted that the small number of e-cigarette users may have limited the ability to find an association between e-cigarette use and quitting.

The report, published online March 24 in JAMA Internal Medicine, also found that women, younger adults and people with less education were most likely to use e-cigarettes.

One expert said the study is flawed and shouldn’t be taken seriously. “It’s an example of bogus or junk science,” said Dr. Michael Siegel, a professor of community health sciences at Boston University School of Public Health.

“That’s because the study does not examine the rate of successful smoking cessation among e-cigarette users who want to quit smoking or cut down substantially on the amount that they smoke, and who are using e-cigarettes in an attempt to accomplish this,” Siegel said. “Instead, the study examines the percentage of quitting among all smokers who have ever tried e-cigarettes for any reason.”

Many of the smokers who tried e-cigarettes may have done so out of curiosity, Siegel said. “It is plausible, in fact, probable, that many of these 88 smokers were not actually interested in quitting or trying to quit with electronic cigarettes,” he said. “These products have become very popular and have gained widespread media attention, and it is entirely possible that many of these smokers simply wanted to see what the big fuss is all about.”

Calling that a “fatal flaw” in the research, Siegel said it “destroys the validity of the authors’ conclusion.” It would be a tragedy, he said, if policy makers use the study to draw conclusions about the effectiveness of e-cigarettes for smoking cessation purposes.

Erika Ford, assistant vice president for national advocacy at the American Lung Association, said the study confirms what is already clear — “e-cigarettes are not associated with quitting among smokers.”

Ford noted that most e-cigarette companies no longer make claims that their products help smokers quit. “But there is a need for the FDA [U.S. Food and Drug Administration] to begin their oversight of these products. It’s time for the FDA to find out which products are making no smoking claims and which ones might be in violation of current law,” she said.

The FDA plans to introduce regulations for e-cigarettes, but hasn’t yet. In the past, the agency has warned companies about making false claims and for poor manufacturing practices.

E-cigarette vapor contains nicotine, not other toxins

People standing near someone using an e-cigarette will be exposed to nicotine, but not to other chemicals found in tobacco cigarette smoke, according to a new study.
E-cigarettes, or electronic cigarettes, create a nicotine-rich vapor that can be inhaled, or ‘vaped.’

Researchers and regulators have questioned whether e-cigarettes are a smoking cessation aid or may lure more young people toward smoking, as well as what effects they have on health.

“There is ongoing public debate whether e-cigarettes should be allowed or prohibited in public spaces,” study co-author Maciej Goniewicz told Reuters Health in an email.

Goniewicz is a cancer researcher in the Department of Health Behavior at the Roswell Park Cancer Institute in Buffalo, New York.

“E-cigarettes contain variable amounts of nicotine and some traces of toxicants. But very little is known to what extent non-users can be exposed to nicotine and other chemicals in situations when they are present in the same room with users of e-cigarettes,” Goniewicz said.

He and his colleagues conducted two studies of secondhand exposure to e-cigarette vapors in a laboratory. Their results were published in Nicotine and Tobacco Research.

In the first study, the researchers used an electronic smoking machine to generate vapor in an enclosed space. They measured the amount of nicotine as well as carbon monoxide and other potentially harmful gases and particles in the chamber.

The second study included five men who regularly smoked both tobacco cigarettes and e-cigarettes. Each man entered a room and smoked his usual brand of e-cigarette for two five-minute intervals over an hour while the researchers measured air quality. The room was cleaned and ventilated and the experiment was repeated with tobacco cigarettes.

The researchers measured nicotine levels of 2.5 micrograms per cubic meter of air in the first study. Nicotine levels from e-cigarettes in the second study were slightly higher at about 3.3 micrograms per cubic meter. But tobacco cigarette smoking resulted in nicotine levels ten times higher at almost 32 micrograms per cubic meter.

“The exposure to nicotine is lower when compared to exposure from tobacco smoke. And we also know that nicotine is relatively safer when compared to other dangerous toxicants in tobacco smoke,” Goniewicz said.

E-cigarettes also produced some particulate matter, but regular cigarettes produced about seven times more. E-cigarettes didn’t change the amount of carbon monoxide or other gases in the air.

“What we found is that non-users of e-cigarettes might be exposed to nicotine but not to many toxicants when they are in close proximity to e-cigarette users,” said Goniewicz.

“It is currently very hard to predict what would be the health impact of such exposure,” he added.

He said more research is needed to find out how the current findings correspond to “real-life” situations, when many people might be using e-cigarettes in a room with restricted ventilation.
