Many of you may have tasted this mouth-watering treat, or have at least seen or heard about it. Guava is very common in Asian countries, but is increasingly available in the western world, particularly as more of its health benefits are revealed. It is a somewhat round or pear-shaped seasonal fruit, and is light green, yellow, or maroon in color on the outside when it is ripe. Guava also has white or maroon flesh and lots of small hard seeds enveloped in very soft, sweet pulp. It is eaten raw (ripe or semi-ripe) or in the form of jams and jellies.

Surgery May Be Best for Young Men With Prostate Cancer

Younger men who have been diagnosed with prostate cancer may do well to consider surgery over so-called “watchful waiting,” a new study shows.

The new research,in the New England Journal of Medicine, is unlikely to end the long-running debate in the medical community over if and when surgery to remove the prostate is needed — particularly since the men in the study were diagnosed before the sensitive prostate-specific antigen (PSA) test was widely implemented to detect prostate cancer in its early stages.

The findings are the latest to come out of a 23-year-long, ongoing study comparing radical prostatectomy versus watchful waiting in 695 men who had been diagnosed with localized prostate cancer.

Between 1989 and 1999, researchers randomly assigned these men to either receive prostatectomy or not. The study found that those younger than 65 who underwent surgery to remove their prostates had a 15.8 percent lower risk of dying from prostate cancer than patients who had not had surgery. They also had a 25.5 percent lower risk of dying from any cause, as well as a 15.8 percent lower risk of the disease having spreading to other organs.

“Our results suggest that surgery may be more beneficial to younger men,” said study author Jennifer Rider, assistant professor of epidemiology at the Harvard University School of Public Health. Rider added that the men under 65 who had their prostates removed were also less likely to need radiation or chemotherapy.

The trial, funded by the Swedish Cancer Society and the U.S. National Institutes of Health, is the first of its kind to entail such an extended follow-up.
Urologists not involved with the research said the findings were important, particularly at a time when many men have concerns about the side effects of going under the knife for prostate cancer.

“This study makes a world of a difference,” said Dr. David B. Samadi, chairman of urology at Lenox Hill Hospital in New York City.
Samadi added that in the years since the study began, the operations to remove the prostate have become more refined.

“Surgery has become much improved,” he said. “ We’ve come a long way.”
“The important finding is that the benefits of surgery have increased over the course of time,” said Dr. Philip Kantoff, director of genitourinary oncology at Dana-Farber Cancer Institute in Boston.

But Dr. Otis Brawley, chief medical officer for the American Cancer Society, said that the results should be interpreted with caution, particularly since the study dealt with men who were not diagnosed using a PSA test – currently the common early detection method used in the U.S.

“That is a very different population from men in the U.S., where PSA screening means most cancers are caught at a much earlier stage,” he said in a statement to ABC News. The concern is that using prostate-removing surgery to treat these very early cancers may not be the best option, as it could mean a drastic surgery to remove a prostate that would never have presented a life-threatening problem.

And Dr. Lee Green, professor of family medicine at the University of Michigan, said more research will be needed before men facing a prostate cancer diagnosis get definite answers to the questions they have.

“It doesn’t prove that radical prostatectomy is better than less radical procedures, and it doesn’t prove that surgery is better than chemotherapy,” Green said. “It just says that surgery is better than doing nothing.”

Prostate cancer chances rise with vitamin E, selenium supplements

The trial was stopped earlier than planned because it showed no protective effect for selenium and suggested an increased risk of cancer from vitamin E. Researchers told the men in the study to stop taking the supplements and kept checking in with them.

Investigators then re-analyzed the data for 1,739 men from the trial who were previously diagnosed with prostate cancer, compared with 3,117 others, without prostate cancer, who were matched to the patients by race and age.

The focus of the research then shifted to how the amount of selenium in the men’s bodies before they started taking supplements affected cancer rates.

The bottom line was there were no benefits to any men from either selenium or vitamin E supplements, said researchers. Taking selenium supplements increased the risk of high-grade cancer among men who had naturally high selenium levels.

“Men using these supplements should stop, period,” study author Alan Kristal of the Public Health Sciences division at Fred Hutchinson Cancer Research Center in Seattle said in a release.

Neither selenium nor vitamin E supplementation offers any known benefits, only risks, he said.

“While there appear to be no risks from taking a standard multivitamin, the effects of high-dose single supplements are unpredictable, complex and often harmful.”

There are still open questions about selenium and prostate cancer risk, according to a journal editorial published with the study. For example, animal studies suggest the form of selenium matters, such as whether it comes from pills, enriched wheat and broccoli, or yeast.

“We need to revisit the question of evaluating the prevention of a single disease in healthy people, where competing risks may obscure reality. Even if a subset can be identified that would benefit from some form of selenium or vitamin E supplementation, it must not be only a benefit to a single gland or based on one disease,” biostatistician Paul Frankel from City of Hope Comprehensive Cancer Center in Duarte, Calif. concluded in the editorial.

Prostate’s Early Growth May Reveal Cures for Later Illnesses

For a surgeon who has successfully treated prostate cancer in many thousands of men by removing their prostate gland, the idea that science might one day be able to regenerate this gland using stem cells is a foreign one — and yet highly intriguing. But this advancement is just one of many potential treatments for prostate cancer or benign prostate enlargement that may eventually arise from important new research on the cellular building blocks of prostate gland development.

In a study published Feb. 11 in the journal Stem Cell Reports, scientists from the University of York in England detailed their discovery of a “signaling pathway,” a set of signals that tell proteins inside stem cells how to evolve into prostate tissue cells called basal cells and luminal cells. The researchers learned there are 80 genes involved in this process, and that the main signals responsible for activating prostate development are retinoic acid and male sex hormones — the balance of which are disrupted in prostate cancer.

A new case occurs every 2.5 minutes and a man dies from prostate cancer every 17 minutes. ‘It has become the second most frequently diagnosed cancer after lung cancer. The conference aims at enhancing the skills of urologists in the performance of advanced urologic endoscopic surgery. The five-day conference will be attended by over 3000 urologists across India, SAARC countries, US, Europe (UK, France, Germany, Spain) and Australia.

A non-smoking man is more likely to get prostate cancer than lung, bronchus, colon, rectal, bladder, lymphoma, melanoma, oral and kidney cancers combined.

Brisk walk can help beat prostate cancer

Exercise may improve the prognosis of prostate cancer patients by affecting blood vessels in their tumours, a study suggests.

Researchers found that men who walked at a fast pace before being diagnosed with the disease had tumours containing larger and more regularly shaped blood vessels.

Better formed tumour blood vessels may in turn inhibit cancer aggressiveness and promote better responses to treatments, the scientists believe.

Physically active men with prostate cancer have a lower risk of recurrence and death from the disease than those living sedentary lives, but until now the reason has remained a mystery.

The new study looked at 572 prostate cancer patients taking part in a US lifestyle and health investigation called the Health Professionals Follow-up Study.

Men with the fastest walking pace – between 3.3 and five miles per hour – prior to diagnosis had 8 per cent more regularly shaped tumour blood vessels than the slowest walkers who ambled at 1.5 to 2.5 mph.

“Prior research has shown that men with prostate tumours containing more regularly shaped blood vessels have a more favourable prognosis compared with men with prostate tumours containing mostly irregularly shaped blood vessels,” said lead scientist Dr Erin Van Blarigan, from the University of California at San Francisco.

“In this study, we found that men who reported walking at a brisk pace had more regularly shaped blood vessels in their prostate tumors compared with men who reported walking at a less brisk pace.

“Our findings suggest a possible mechanism by which exercise may improve outcomes in men with prostate cancer. Although data from randomised, controlled trials are needed before we can conclude that exercise causes a change in vessel regularity or clinical outcomes in men with prostate cancer, our study supports the growing evidence of the benefits of exercise, such as brisk walking, for men with prostate cancer.”

Dr Matthew Hobbs, deputy director of research at Prostate Cancer UK, said: “Although this research provides a plausible explanation of how exercise might improve outcomes for men with prostate cancer, much more research is needed to confirm the impact of lifestyle factors on men’s recovery.

“We hope that further research in this area may one day give us a way to improve the prognosis for the 40,000 men in the UK who are diagnosed with prostate cancer each year.”

Meanwhile a separate study suggest a good night’s sleep may also held the condition. Scientists have linked higher levels of the night-time hormone melatonin with a 75 per cent reduced risk of advanced disease.

Melatonin is produced in the dark at night. It plays a key role in regulating the body’s sleeping cycle and influences many other functions associated with the body’s 24-hour clock, or circadian rhythm.

Low levels of the hormone are typically associated with disrupted sleep.

Scientists studied 928 Icelandic men who were questioned about their sleep patterns.

Grape seed Extract Kills 76% of Leukemia Cancer Cells in 24 Hours

A new study by Indian origin researcher has revealed that the synthesis of the most active component of grape seed extract, B2G2, encourages the cell death known as apoptosis in prostate cancer cells while leaving healthy cells unharmed.

“We’ve shown similar anti-cancer activity in the past with grape seed extract (GSE), but now we know B2G2 is its most biologically active ingredient which can be synthesized in quantities that will allow us to study the detailed death mechanism in cancer cells,” Alpna Tyagi, PhD, of the University of Colorado Skaggs School of Pharmacy and Pharmaceutical Sciences, said.

Previous studies have shown the GSE effectiveness against cancer cells and have also shown its mechanism of action. However, Tyagi said that until recently, they didn’t know which constituent of GSE created this effect. This naturally occurring compound, GSE, is a complex mixture of polyphenols and also so far it has been unclear about the biologically active constituents of GSE against cancer cells.

Eventually the group pinpointed B2G2 as the most active compound, but, “it’s expensive and it takes a long time to isolate B2G2 from grape seed extract,” Tyagi says.

The current study reported the success of this effort, including the ability to synthesize gram-quantity of B2G2 reasonably quickly and inexpensively.

The study also showed anti-cancer activity of synthesized B2G2 similar in mechanism and degree to overall GSE effectiveness.

The study was published in the journal Nutrition and Cancer.
