Diabetic women should monitor glucose levels before pregnancy

Women suffering from diabetes and wishing to start a family should monitor their blood glucose levels and take a daily high dose of folic acid before pregnancy to avoid risk to their baby’s health, a new study said on Wednesday. 

The risk of stillbirth – when the foetus dies in the uterus or death during the first year of birth – was over four times greater in women with diabetes than in those without the disease.

The team from Newcastle studied the outcome of over 400,000 pregnancies delivered in north of England between 1996 and 2008.

”We found that 2.7 percent of births in women with diabetes resulted in stillbirth, six times than the rate for women without diabetes, while 0.7 percent died during the first year of life, nearly double the rate in women without diabetes,” said Ruth Bell, one of the researchers.

The research also said that nearly 40 percent of deaths might have been avoided if all of the women were able to achieve good control of their blood glucose before pregnancy.

”Stillbirths and infant deaths are thankfully not common, but they could be even less common if all women with diabetes can be helped to achieve the best possible control of their blood glucose levels before becoming pregnant,” added Bell.

Lifestyle factors linked to healthy pregnancy

Researchers have identified certain lifestyle factors that make it more likely for a woman to have a normal pregnancy.

They include increasing fruit intake before pregnancy, being a healthy weight, reducing blood pressure, stopping drug and alcohol misuse, and being in paid employment during pregnancy.

Although further work is needed to determine whether these associations have causal importance, this study implies that targeted interventions that encourage women to make healthy choices before and during pregnancy “may increase the likelihood of normal pregnancy outcomes,” the researchers said.

Researchers from the UK, Ireland and New Zealand carried out a study to identify factors at 15-20 weeks’ gestation associated with a subsequent uncomplicated pregnancy. Their aim was to highlight those factors amenable to change before pregnancy, thereby informing decisions about interventions that could increase the likelihood of a normal outcome.

A total of 5,628 healthy women with singleton births (and no previous pregnancies) were recruited to the Screening for Pregnancy Endpoints study between November 2004 and August 2008.

The primary outcome was uncomplicated pregnancy defined as a “normotensive pregnancy, delivered at more than 37 weeks resulting in a live born baby who was not small for gestational age and did not have any other significant pregnancy complications.”

Factors amenable to improvement that reduced the likelihood of an uncomplicated pregnancy (that is, were detrimental) were increasing body mass index and blood pressure and misuse of drugs (including binge drinking) in the first trimester.

Factors amenable to improvement that increased the likelihood of an uncomplicated pregnancy (that is, were beneficial) were high fruit intake in the month before pregnancy and being in paid employment 15 weeks into pregnancy.

Detrimental factors that could not be altered were being in a lower socio-economic group, high blood pressure before pregnancy while taking oral contraceptive pills, family history of high blood pressure in pregnancy and bleeding during pregnancy.

Exercise can help kids stave off negative effects of maternal obesity

Researchers have found that off springs whose mothers were fed a high-fat diet during pregnancy and nursing were able to stave off some of the detrimental health effects of obesity by exercising during their adolescence.

It was found that even though the rat offspring weighed the same as their sedentary counterparts, the exercising rats had fewer fat deposits and their brains were better able to respond to a hormone known to suppress the appetite, long after they stopped running on their exercise wheels.

Because mammals (including rats and humans) share much of their biology, the findings suggest that childhood exercise might help mitigate some of the risks that human children of obese parents are biologically primed to follow in their parents’ footsteps and to develop diabetes and other obesity-related disorders.

“Just three weeks of exercise early in life had a persistent effect on the satiety centres of the brains of these rat pups,” study’s lead author Kellie L. K. Tamashiro, Ph.D., an associate professor of psychiatry and behavioral sciences at the Johns Hopkins University School of Medicine, said. “If we can find a way to take advantage of th

at phenomenon in humans that would be great, because preventing obesity is probably going to be much easier to do than reversing it.”

Tamashiro and her colleagues fed pregnant rats a high-fat diet and continued that diet while they were nursing their pups. The animals were weaned on a healthier, standard low-fat diet and at four weeks of age, the equivalent of rodent early adolescence, some were given free access to running wheels in their cages, while the others remained sedentary.

To determine the impact of the exercise on appetite, at 14 weeks of age, the rats’ brains were injected with the appetite-suppressing hormone leptin. Those that had exercised weeks before ate less, while the sedentary rats showed no differences in their appetites.

Egg Timer Test proves unreliable guide to fertility

A popular fertility test designed to tell a woman how long she has left to fall pregnant is providing inaccurate and misleading results, creating a wave of panic among women in their 30s and 40s, Australia’s leading fertility expert, Dr Anne Clarke, said.

Dr Clarke, medical director of Fertility First in Sydney, said a recent British study, plus anecdotal evidence, had found the simple blood test, known as the Egg Timer Test, was unreliable and becoming discredited worldwide. ”I have big concerns about its accuracy,” she said. ”I’m seeing a lot of women turning up at my clinic in an incredibly distressed state and highly depressed because they’ve been told the test showed they had no chance of having a baby. It’s wrong and misleading.”

Among them was a 40-year-old Sydney woman who was told by her GP in April last year that the test, which measures the level of anti-mullerian hormone (AMH) in the blood, showed her ovarian reserve was dangerously low. Further analysis revealed she was very fertile and well within the normal range, Dr Clarke said.

The Egg Timer Test – which costs about $70 – was pioneered by Adelaide clinic Repromed in 2004, to measure the number of eggs a woman had and predict how many child-bearing years she had left. With thousands of women rushing to take the test, other companies entered the market, but Dr Kelton Tremellen, of Repromed, said they were not always reliable. ”[If it’s not done properly] one person’s blood test can be analyzed and get two vastly different results,” he said. Results could be compromised, for example, if a woman had been on the pill. Dr Clarke added that inaccurate readings also occurred when the blood was stored incorrectly or the hormone not analyzed immediately.

 

Asked if women should have the test, she said: ”I’m not sure of the value of the test. If I want to look at ovarian reserves, I do an antral follicle count with an ultrasound.”

The first reported study on the effectiveness of the Egg Timer Test was damning. The Manchester study, published last year, found significant variations in the results – up to 60 per cent.

Head researcher Dr Oybek Rustamov said the study, which looked at the results of 5000 women between 2008 and 2011, found ”commercial AMH or Egg Timer Tests provide erroneous results”.

Dr Clarke said research was increasingly discrediting and devaluing the test as a means of gauging a woman’s biological clock.

Cheriece Harper, 31, from Penrith, had the Egg Timer Test in 2011 and was left depressed when her doctor told her she had little chance of conceiving. Ms Harper consulted Dr Clarke, became pregnant via a sperm donor and gave birth to Bridie in October last year.

”I’m glad I had the test because it pushed me to make a decision and not delay motherhood, but if women get it done, they need to know it’s measuring egg quantity, not quality.”

Exercise may help pregnant women quit smoking

For pregnant women who want to quit smoking, a brisk walk can temporarily stave off tobacco cravings, says a Canadian study.

Previous research has shown that exercise can interrupt nicotine cravings for both men and women. Whether the same was true for expecting mothers was unclear because pregnant women have increased metabolism, which can intensify longings for a cigarette, the researchers write in the journal Addictive Behaviors.

“This was the first time we have been able to replicate the findings with pregnant smokers,” Harry Prapavessis said.

Prapavessis, director of the Exercise and Health Psychology Laboratory at Western University in Ontario, Canada, led the research.

According to his team’s results, 15 to 20 minutes of walking at a mild to moderate pace is sufficient to ward off cravings.

For the study, researchers recruited 30 pregnant women in their second trimester in Canada and England. All of the women smoked more than five cigarettes a day and were not regular exercisers.

Half of the women were assigned to walk on a treadmill and the rest to watch a home gardening video for 20 minutes. Both groups did not smoke for between 15 and 19 hours before entering the lab.

The walkers reported an average 30 percent reduction in the desire to smoke based on a seven-point scale. But the cravings returned. Thirty minutes after exercising, the same group of women reported only a 17 percent craving reduction.

The exercising women also reported less irritability, restlessness, tension and other withdrawal symptoms. But because of the study’s small size, those results could have happened by chance.

“This translates not as a cure for quitting, but it can be part of a strategy,” said Dr. Sharon Phelan, who was not involved in the study.

Phelan is a fellow with the American Congress of Obstetricians and Gynecologists (ACOG) and professor at the University of New Mexico School of Medicine in Albuquerque.

“The challenge is that there isn’t one reason why pregnant women have an addiction,” Phelan said.

“I think it’s a very positive study,” said Dr. Raul Artal of Saint Louis University School of Medicine in Missouri. Artal helped write exercise and pregnancy guidelines for ACOG.

He said the new study will need to be repeated according to medicine’s gold standard of testing – a randomized, controlled trial. “But, based on common sense, the message is good,” he said.

Prapavessis said his team’s results can only be applied to women about 25 years old, the average age in the study. But, “I would like to think that we can repeat the findings with older or younger pregnant smokers.”

Prapavessis pointed out that because of the social stigma associated with smoking while pregnant, recruiting pregnant women for such studies can be extremely difficult.

The next step, he said, would be to repeat the results with women walking in natural environments outside of their homes. “We want to see if this craving effect can be reproduced when women go for a brisk walk for about 15 minutes in their natural setting,” he said.

Pregnant women also have the option to try nicotine replacement therapy drugs, like skin patches or lozenges, but more evidence is needed to know if these are completely safe during pregnancy, Phelan said.

To help pregnant smokers quit, Phelan stressed the importance of understanding the underlying reasons why a pregnant woman smokes. “It’s like when someone has a fever. You can treat it with an aspirin, but you haven’t gotten to the underlying cause.”

“One therapy for everyone isn’t going to fit all,” Phelan noted. Still, regardless of whether a woman has stopped smoking, exercise offers positive benefits, like improved circulation and muscle tone she said.

ACOG supports 30 minutes of light exercise like walking three or four times a week during pregnancy, Phelan said, but pregnant women should always talk with their healthcare provider before beginning exercise.

To beat cigarette cravings, she said, “This is a valid option to suggest to women and it may be helpful for some, but not to others.”

Eating tips to boost fertility revealed

Women watching their weight and closely following a Mediterranean-style diet that is high in vegetables, vegetable oils, fish and beans may boost their chance of becoming pregnant, according to dietitians.

Brooke Schantz, MS, RD, CSSD, LDN, LUHS, said that establishing a healthy eating pattern and weight is a good first step for women who are looking to conceive.

She said that not only will a healthy diet and lifestyle potentially help with fertility, but it also may influence foetal well-being and reduce the risk of complications during pregnancy.

Schantz said that reduced intake of foods with trans and saturated fats while increasing intake of monounsaturated fats, like avocados and olive oil could help women who are looking to conceive.

Another tip was lower intake of animal protein and adding more vegetable protein and fibre to their diet.

She also said that incorporating more vegetarian sources of iron like legumes, tofu, nuts, seeds and whole grains may help women in their endeavour.

Drinking milk in pregnancy helps kids gain height

A new study has found taking milk during pregnancy benefits children well into their early adulthood. It helps the children gain better height than those whose mothers did not take it adequately. Earlier studies said babies tend to weigh more and grow more quickly if expectant mothers drink milk. But the new study has said children born to women who drank milk during pregnancy are more likely to be tall in their teenage. The study`s results, published in the European Journal of Clinical Nutrition, show teenagers of both genders were generally taller if their mothers took more than 150 millilitres – roughly a quarter of a pint of milk – a day during the pregnancy, compared to children born to women who drank less than that quantity, Daily Mail reported. The latest finding was reached after a team of scientists, who tracked babies born in the late `80s, found their height during adolescence was directly related to how much milk their mothers consumed when they were in the womb. Nutrition experts from Iceland, Denmark and the US wanted to examine if the benefits seen in the early stages of life from milk were extended into later years. They tracked babies born to 809 women in Denmark in 1988 and 1989 by monitoring how much milk the women had consumed during pregnancy. The babies were measured for weight and birth length and then followed up again almost 20 years later.
